GRx Patient Support
GRx Support Services at Harbour Sport
GRx client support forms the hub of information on activities and contact on what is happening in each local area in the North Harbour and Rodney regions. This support enables GRx clients to have ongoing advice and encouragement over the period of their GRx.
At Harbour Sport we have a team of four GRx PSPs: AJ Jones, Jenny Lim, Liz Golding, and Nesha Greasley. To find out more about the GRx PSPs click staff page. The qualified team with Carl Fenton (Green Prescription/Active Families Manager), work with activity providers to ensure that all patients have the best possible opportunities to become active in a safe and fun environment, while encouraging sustainable lifestyle changes of their choice.
GRx modes of support offered at Harbour Sport
Phone Support
(GRx clients receive phone support for 3-4months) GRx clients assigned a Patient Support Person (PSP). PSP will provide support in the form of advice, motivation and provide GRx clients with information about the physical activity opportunities in their community. As an incentive to encourage GRx clients to increase their physical activity, discounts are offered at various gyms/ recreation centres and community exercise groups, GRx clients can access these discounts with their GRx. 1st Phone call: PSP will ring their client within 5 weeks of receiving their script/ the referral. This phone call is an opportunity for the PSP to introduce themselves, explain the GRx process, get to know their client, discuss physical activity options and set goals. Following the 1st call an information pack is sent to the GRx client containing information about activity providers, goal setting sheet, nutritional information and any additional information required. 2nd Phone call: The 2nd phone usually occurs within 4 weeks of receiving their GRx information pack. This phone call provides PSPs and GRx clients with an opportunity to discuss progress, re-visit physical activity goals and PSPs provide additional motivation and encouragement. If any additional information is required the PSP will post this to the GRx client, or they are encouraged to attend the GRx clinic. 3rd Phone call: The 3rd phone call occurs 4-6 weeks after the 2nd phone call. This is generally the final call, the main purpose of this call is to establish whether or not GRx client is sufficiently active (Physically active on 5 or more days/ week, 30mins/ day). If the client is sufficiently active they will be congratulated on their achievement, encouraged to continued being physically active and discharged from GRx. If the client is insufficiently active (Physically active for less than 30mins on less than 5 days/ week) they are encouraged to get a second GRx and discharged from GRx. Upon discharge letter is sent to the GRx clients referrer summarising their progress.
Face to Face Support GRx Northcote Community Programme
The aim of the Northcote GRx Community Programme is to equip GRx clients with the, knowledge and tools to lead a physically active lifestyle.
The Northcote GRx community group provides GRx clients with 6months of support. The group meets every Tuesday, 10am –11.30am during the school term. The first visit is FREE and $3/ visit thereafter. The following is an overview of the structure: 10am – 10.15am: Registration 10.15am – 10.45am: Education session, providing participants with information on the following; physical activity options, healthy eating/ nutrition and goal setting/ motivation. 10.45am – 1130am: Activity session, activities include; bush walks, circuits, low impact aerobics and fun games.
Every 6 months participants will be given the opportunity to attend an orientation session at the following fitness centres; YMCA North Shore, AUT Sport & Fitness Centre and North Shore Leisure Glenfield.
Once a month participants complete a review to track their progress.
Upon graduation participants receive a 1 months free membership to one of the following centre (YMCA North Shore, AUT Sport & Fitness Centre and North Shore Leisure Glenfield). In addition, participants are encouraged to become a volunteer for the community programme and continue being involved. Day: Every Thursday during the school term Time: 10am-11.30pm Venue: Beach Haven Rate Payers Hall (Behind the Chemist) (366 Rangatira Road, Beach Haven) What we offer? 6months face to face support for GRx clients in Beach Haven and Surrounding areas 45min EDUCATION session covering the following: • Goal Setting • Motivation • Benefits of physical activity • Activity options • Healthy Eating • To name a few... 50-60min FUN ACTIVITY session An orientation day at Beach Haven Sports Centre once a term Monthly reviews to progress The group will meet every Thursday, 10am –11.30am during the school term.
